Overview COBRA Rules:

• You must return your initial election form within 60 days of the loss of coverage
• You must return your first payment within 45 days of returning your election form
• Coverage will not begin until BOTH the enrollment form AND your payment have been received
• Monthly payments are due the 1st of the month and you have a 30 day grace period
• Coverage will be terminated if payments are not received on time
